What Are Home Care Packages?
Home Care Packages are government-funded programs designed to provide coordinated packages of care and services to help older Australians remain living independently in their own homes for as long as possible.
Four Levels of Support
HCP are available at four levels, from basic care needs (Level 1) to high care needs (Level 4). Each level receives different funding amounts to match the required care intensity.
Services Included in HCP
- Personal care (showering, dressing)
- Domestic assistance (cleaning, laundry)
- Meal preparation and nutrition
- Home maintenance and modifications
- Transport to appointments
- Social support and activities
- Nursing and allied health
- Case management
Eligibility & Assessment
To access a Home Care Package, you need to be assessed by the Aged Care Assessment Team (ACAT). Eligibility is based on your care needs, not your income or assets.
Assessment Process
- Contact My Aged Care (1800 200 422)
- Complete a phone assessment
- Face-to-face assessment by ACAT if needed
- Receive approval and package level
- Choose your preferred provider (like us!)
